New Patients

Welcome to Lakewood Family Dental Care, Family and Cosmetic Dentistry in Dallas, Texas. We will need some information to expedite your first visit to our dental office.

There are two ways to make this easy for you:

1. You can print out the New Patient Information/Medical History form, from our website, fill it out at your leisure and bring it with you.
   
   
2. The other option is to complete the forms on-line and submit them to us in advance of your appointment.  Our hope and efforts are to make your visits to our office as easy as possible.
